Diablo Adult Education is seeking a qualified, part-time Student Support Services Specialist to work with individual students and their caregivers in Mt.
Diablo Adult Education's On Track Program.
On Track is a model education program serving adults with acquired or traumatic brain injury (due to head trauma, stroke, brain tumor, etc.
).
The Student Support Services Specialist works collaboratively with the On Track instructional team, local businesses, and community organizations to provide the following:
? Identification of needs and referrals to services outside of the program ? Individual support to students that may include volunteer or job placement and coaching; setting up support systems for housing and independent living; and individual/group travel training to access resources/services ? Resource development and support for caregivers/families as needed (referrals, caregiver resource groups) ? Documentation of needs assessments, service plans, and progress notes ? Outreach to individuals in the community with acquired or traumatic brain injury Job Type:
